Medication Management Policy

Client Responsibilities for Medication Appointments

 

  • Timely Scheduling: Schedule appointments at least a week before needing a refill to ensure continuous medication management.
  • Advance Notice: Provide at least 24 hours' notice for cancellations or rescheduling.
  • Attendance: Attend scheduled appointments. Missed appointments may affect continued care; running out of medication due to missed appointments is not considered an emergency.
  • Medical History: Provide accurate medical history and updates on health changes during appointments.
  • Medication Records: Bring a current list of all medications (prescription, OTC, and supplements) to each appointment.
  • Informing Changes: Report any health changes, medication side effects, or issues with the current regimen.
  • Follow-Up: Complete follow-up tasks, including testing, referrals, or recommendations.
  • Following Instructions: Adhere to prescribed medication instructions and report any concerns or side effects promptly.
  • Emergency Contact: Contact your provider or seek emergency help for severe medication-related side effects.
